The Network Access Plan option allows individuals to purchase a minimum level of dental benefit while tapping into maximum network savings that are not normally offered outside of employer-sponsored plans.
The plan is tied to a network of participating dentists who have signed contracts and agreed to accept certain fees for dental services. A member must visit a dentist in the network to enjoy the contracted savings. Use the Find a Dentist tool to review a listing of network dentists in your area.
Individuals must live in Kentucky, Indiana or Ohio (Lucas & Wood Counties only) but network access is available in all 50 states.
Your $100 annual benefit will be applied to covered services and paid directly to your dentist upon the dentist's submission of your claim. To verify appropriate charges and savings, an Explanation of Benefits (EOB) will be provided to you and your dentist upon submission of a claim.
